NON-OPERATIVE METHODS (without operation ): These are techniques used to treat piles without surgery. Procedures can be done on day care basis , with less pain and inconvenience . These methods are ideal only for treating internal piles. External piles requires surgery.
Following Methods are Popular :
SCELEROTHERAPHY: A chemical is injected to block the blood vessel causing the piles, so that it prevents bulging and bleeding from piles while passing stools. Very effective in initial stages and totally painless
BANDING: A tight rubber band is applied to the base of the internal pile using a special instrument. Such banded internal pile falls off over a period of 7-10 days, resulting wound heals on its own. Very effective for second and early third degree piles. Mild pain is experienced for a few days .
CRYO-SURGERY: Even though this method is not practiced widely , finds favour with some proctologists. Here intense cold probe is applied to the pile which freezes it. This is nothing but cold burn. Over a period of 7-10 days internal pile falls off , resulting wound will heal subsequently. Mild to moderate pain may follow which will be relived with analgesics.
INFRARED COAGULATION: Infrared rays are used to burn the blood vessel causing the pile. Technique similar to scelerotherapy.
STAPLER SURGERY : Recently introduced technique where internal piles are pulled up, cut and sutured at one go , using a disposable instrument using staplers. .
LASER SURGERY : Instead of surgical knife, laser is used to perform the procedure. Studies have found out that there is no significant reduction in post operative pain compared with regular surgery.
CLASSICAL SURGERY : Gives very good results but post operative pain is an issue. With advent of good analgesics , post operative agony can be reduced significantly.
Classical surgery should be reserved for advanced and complicated piles and those which do not respond to non operative procedures.
